A Comprehensive Checklist for Families

Hiring a private autopsy service is a decision most families face only once in their lives, often during the worst moments. Without experience in evaluating forensic pathology providers, it is easy to overlook critical questions that can affect the quality of the examination, the usefulness of the results, and the total cost. This checklist provides every question you should ask before committing to a private autopsy provider, organized by category to help you cover all the essential bases even under time pressure.

Credentials and Qualifications

  • Are you board-certified in forensic pathology by the American Board of Pathology?
  • How many years have you been practicing forensic pathology?
  • Approximately how many autopsies have you performed in your career?
  • Do you hold a current, unrestricted medical license in this state?
  • Are you a member of the National Association of Medical Examiners or other professional organizations?
  • Have you had any disciplinary actions against your medical license?
  • Do you have specific experience with cases similar to mine? (Specify: overdose, infant death, suspected abuse, medical error, etc.)

The Examination Process

  • Will you personally perform the autopsy, or will another pathologist or assistant conduct part of the examination?
  • What does your standard autopsy include? (Full internal examination, external examination, organ weights, microscopic tissue examination?)
  • What photographic documentation is included? Will I receive copies of photos if needed?
  • Do you perform radiographic imaging (X-rays or CT) as part of the examination?
  • Where is the autopsy performed? Can you describe the facility?
  • How do you maintain chain of custody for evidence and specimens?

Testing and Analysis

  • What toxicology panel is included in the standard fee? How many substances does it screen for?
  • Do you test for fentanyl analogs, novel psychoactive substances, and other emerging drugs?
  • Is histological (microscopic tissue) examination included?
  • What specialized testing might be recommended for my case, and what would it cost?
  • Which laboratory processes your toxicology and histology specimens? Are they accredited?
  • Can you arrange molecular autopsy (genetic testing) if the conventional autopsy is inconclusive?
  • What specimens do you routinely retain, and for how long?

Timeline and Communication

  • How soon can the autopsy be scheduled?
  • How long does the physical examination take?
  • When can I expect preliminary verbal findings?
  • What is your typical turnaround time for the final written report?
  • Do you offer expedited reporting for cases with legal or insurance deadlines?
  • Will you be available to discuss the findings with our family after the report is complete?
  • Who should I contact if I have questions during the process?

Cost and Payment

  • What is the total fee for the standard autopsy examination?
  • What is included in that fee? (Examination, toxicology, histology, report?)
  • What additional costs might arise? (Specialized testing, extended toxicology, neuropathology, transport?)
  • Is body transport included, or is that a separate expense?
  • When is payment due? Do you require payment before the examination?
  • Do you offer payment plans or financial assistance?
  • Will you provide an itemized invoice that can be submitted to insurance or included in legal proceedings?
  • If the case involves potential litigation, can the cost be advanced by an attorney?
  • Have you testified as an expert witness? In how many cases?
  • Have you testified in this state or jurisdiction before?
  • What is your hourly rate for testimony and legal consultation?
  • Will your report be written to withstand legal scrutiny?
  • Do you have any conflicts of interest related to this case? (Employment or financial relationships with the hospital, medical examiner, or other parties involved?)
  • Are you willing to support your findings through deposition and trial testimony if needed?

Logistics and Coordination

  • Can you coordinate directly with our funeral home for body transport?
  • Will the autopsy affect our ability to have an open-casket viewing?
  • How long will you need the body? When will it be returned to the funeral home?
  • What information do you need from us to begin? (Medical records, medication lists, circumstance details?)
  • What authorization forms are required, and who needs to sign them?
  • If the medical examiner has already performed an autopsy, can you still perform a second examination?

After the Examination

  • What format is the final report? How is it delivered?
  • How many copies of the report are provided?
  • Are microscopic slides, tissue blocks, and photographs available for review or to share with other experts?
  • How long do you retain records and specimens?
  • If new questions arise after the report is issued, will you be available for follow-up consultation?
  • Can you provide a supplemental report if additional testing is performed later?

Using This Checklist Effectively

You do not need to ask every question on this list. Focus on the categories most relevant to your situation. If your case is straightforward and unlikely to involve litigation, the credentials, examination, timeline, and cost sections are most important. If your case may lead to legal proceedings, the legal considerations and chain of custody questions become critical.

A reputable forensic pathologist will welcome these questions and provide clear, complete answers. If a provider seems annoyed by your questions, is evasive about credentials or costs, or pressures you to make a decision without adequate information, consider that a warning sign and continue looking for a provider who respects your right to make an informed choice.

Print this checklist, keep it on your phone, or share it with a trusted family member or friend who can help you evaluate providers during what is undoubtedly one of the most difficult periods of your life. Having the right questions ready ensures that you select a provider who will deliver the thorough, professional examination your loved one deserves.
